Getting care in Alabama without insurance

The short answerAlabama has not expanded Medicaid. Most adults without children cannot get it, even with a very low income, unless they are elderly or disabled. You can call Alabama Medicaid at 1-800-362-1504, find a community health center that charges by income, or look at direct primary care.

What Alabama Medicaid is and who can get it

Medicaid pays for health care for people with low incomes. In Alabama, the Alabama Medicaid Agency runs it. The agency says over a million people a year qualify for full or partial Medicaid, and who qualifies depends on income and other rules.

The rules are narrow for working-age adults. Children and pregnant women can qualify at higher incomes than adults can. Parents with children at home can qualify only at an extremely low income. That income is far below the federal poverty level, which is the yearly income the government uses to count who is poor. Adults without children usually cannot get Medicaid unless they are elderly or have a disability.

The exact income limits change, and the sources we read did not agree on the numbers for parents. Please ask the agency for the current limit instead of relying on a number you find online.

Alabama and Medicaid expansion

Alabama has not expanded Medicaid. Expansion means covering more adults with low incomes, and many states have chosen it. KFF, a health policy research group, lists Alabama as a state that has not done it, as of October 2026.

How to apply for Alabama Medicaid

Alabama does not charge a fee to apply, so it is worth trying even if you are not sure you qualify. You have these ways to start.

  • You can call the Alabama Medicaid Recipient Call Center at 1-800-362-1504. The line is open on weekdays.
  • If you are an adult under 65 and not disabled, you can also apply through HealthCare.gov. If you look eligible, your information is passed on to Alabama Medicaid.
  • You can send a paper application by mail or fax, or hand it in at a Medicaid district office. The state has eleven district offices, and each one has its own toll-free number.
  • The agency also lists an online application for children and for pregnancy on its website.

Have proof of your income ready, along with Social Security numbers for everyone who is applying.

Family planning and cancer screening help

The agency has an application for a program called Plan First Family Planning. It is listed next to the pregnancy and children's applications. Ask the call center whether you might qualify.

Alabama also runs a Breast and Cervical Cancer Early Detection Program. You can reach it at 1-877-252-3324, or you can ask your county health department. A county health department is the local public health office that serves your county.
