How to get Medicaid or low-cost care in Iowa

Iowa Medicaid and the Iowa Health and Wellness Plan

Medicaid is a program that pays for health care for people with low incomes. Iowa Medicaid is run by Iowa Health and Human Services, which people shorten to Iowa HHS. The state describes it as a joint program of the federal government and the state.

Iowa began covering more low-income adults on January 1, 2014, according to KFF, a health policy research group. The state calls its adult plan the Iowa Health and Wellness Plan. It is for Iowans ages 19 to 64 who are U.S. citizens. If you have a green card or another immigration status, you can still apply and let the state decide. You must also live in the state and not qualify for another Medicaid group or for Medicare.

The income limit is 133 percent of the federal poverty level, which is a yearly dollar amount the government uses to measure poverty. The state lists that as about $21,227 for one person and about $28,782 for two people, from April 1, 2026. Larger households have higher limits that the state also lists. What the plan costs

Coverage can be free if you complete healthy behaviors. These are simple steps that the plan lists, such as a yearly checkup. Depending on your income, a small monthly payment may start in your second year of coverage. You can pay online through the services portal or by mail. If a payment seems hard to make, call the number below before you miss it.

The Iowa work rule begins December 1, 2026

Iowa calls its work rule community engagement requirements. Officials say the rules begin on December 1, 2026, for people who apply on or after that date. Members who are already enrolled must meet them at their first renewal, and the state has said those renewals start in early 2027.

Only certain members of the Iowa Health and Wellness Plan have to follow it. The federal rule asks for 80 hours a month of work, service or school. The list of approved activities includes employment and school, as well as job training and volunteering.

The state excuses several groups from the rule. One group is people who are pregnant or had a baby in the last 12 months while on Iowa Medicaid. Another is people who care for a child 13 or younger or for someone with a disability. People in a drug or alcohol treatment program are excused. So are people who were recently in jail or prison and people with a disability or serious medical condition. Veterans with a total disability rating are excused as well. American Indians and Alaska Natives are excused, and so are people who already meet SNAP or FIP work rules.

If your income is above the limit

Iowa residents shop on the federal marketplace at HealthCare.gov. A marketplace plan is private insurance that you buy yourself, and a tax credit can lower the monthly cost if your income qualifies. You can shop for 2027 plans from November 1, 2026. Pick a plan by December 15 if you want it to begin on January 1. Help from HealthCare.gov is available by phone at 1-800-318-2596. Community health centers in Iowa

Federally funded community health centers charge by income and family size, and they serve everyone. To find one near you, type your zip code into the free Find a Health Center tool from the Health Resources and Services Administration. Ask about the sliding fee, which is a price scale based on what you earn. Many centers also offer dental and counseling care, so ask what they have.
